Here is the old Los Pantalones application. In retrospect there are a few questions i would add.
    1)where do you live?
    2)would you be able to drink with pants on a predetermined night?
    3)what is your longest kill streak on CSS?
    4) are you considered a robot?
    5)how many licks does it take to get to the center to a tootsie pop?
    6)rock paper or scissors?
    7)how many roads does a man walk down before he is known as a man?
    8)main vice?
    9)married, dating or otherwise?
    10) new england clam chowder is A: red B:white 
    11)wheres the beef?
    12) do you wear pants while playing? (might be a trick question*)
    13) and finally what would you do for a klondike bar?
